Your ZaneHealth benefit reimburses you for your eligible health insurance premiums. You can also request reimbursement for out-of-pocket expenses for basic preventive care.

In order to get reimbursed for this type of expense, please provide: 

  1. Documentation showing the person receiving service, date of service, type of service, provider name and the amount paid,
  2. Documentation showing the expense falls within the definition of basic preventive services as defined in Public Health Service Act Section 2713 (see list below), and
  3. Documentation showing the expense was not covered by a major medical health insurance or another health plan. This information is easiest to find on an Explanation of Benefits (EOB) you can get from your insurance carrier.
